In short
Copy medication information from the current label or veterinary instructions. Do not rely on the color or shape of a tablet, and do not change instructions while organizing the list.

Record each item consistently
- Medication or supplement name exactly as shown on the label.
- Instructions exactly as supplied, including route and timing language.
- Start date, known end date, prescribing clinic, and dispensing pharmacy.
- Amount remaining and the label’s refill information when available.
Include non-prescription products
Include supplements and other products your pet receives so the veterinary team sees the complete list. The list is a record, not an assessment of whether products should be used together.
Check the list before a visit
Compare the list with the physical containers or recent label photos. Mark anything unclear and contact the prescribing clinic or pharmacy rather than guessing.
