Related days, grouped into one concern you can watch.
When the same thing keeps coming up, group the records under one Health Concern and mark it Watching, Stable, or Resolved as it moves. The Monitor screen lists what is being watched and the findings behind it.

Step 1. One thread for one problem
Three scratches, a red ear, and a new pill belong together. A concern holds them, with the first date and the last, so “has it happened before?” has an answer.
Step 2. Watching, Stable, Resolved
Three states, set by you. Watching keeps it on Monitor. Resolved keeps the history and takes it off the front.
Step 3. Findings, worded plainly
Monitor points out what the records show, such as a sign recurring after a food change, in plain words. It never scores or diagnoses.
