Coordinate pet medications at home

When medication care changes hands, the next person should not have to reconstruct the evening from a bottle, a text message, and memory. Give them one short note that shows what was recorded, what comes next, and what is still uncertain.

The five things the next caregiver needs

  • Your pet’s name and the date and time of the handoff.
  • The medication name and instructions copied from the current label or clinic schedule.
  • What was recorded as given, when it happened, and who recorded it.
  • What is scheduled next, including any food or storage instruction from the label.
  • The remaining supply, anything uncertain, and the prescribing clinic or pharmacy contact.

A completed handoff

This is the level of detail another caregiver can use without sorting through a message thread. The medication and timing below are examples, not instructions for another pet.

Pepper’s Wednesday evening handoff

Sam handed care to Jordan at 7:10 p.m.

Recorded: amoxicillin-clavulanate given at 6:30 p.m. by Sam, following Pepper’s current label.

Food note: Pepper ate about half of dinner first.

Next: Thursday morning, as written on the current clinic schedule.

Supply: four tablets remain.

Uncertain: none. Observation for the record: loose stool at 7 p.m.

Use it in the moment

  • Record an administration after it happens. Do not mark it ahead of time.
  • At handoff, check the last record and the next scheduled time together.
  • If a dose was missed, spit out, vomited, or may have been given twice, write down exactly what you know and contact the veterinarian for medication-specific direction.
  • When instructions change, replace the current handoff note so an older version is not mistaken for the active one.
