Use four simple groups
- Baseline profile: what is usual for your pet.
- Official documents: clinic records, test results, certificates, and discharge instructions.
- Current care: medications, reminders, diet, and routine.
- Timeline: visits and owner-observed changes in date order.

Keep originals connected
An owner summary should identify the clinic and date connected to an official document. Preserve the original file or request it from the clinic rather than copying a result without its context.

Review the record at useful moments
Update the record after a visit, when instructions change, and before travel or a new clinic appointment. A short current record is more useful than a large archive no one can scan.
